Pay Price Guarantee: If you find another private for profit clinic that will provide these services for a cheaper price we will price match that price and give you a 10% discount. Just tell the front desk you have a Pay Price Guarantee!
Additional Fees:
$150 dollar fee for transportation and travel costs for any at home visits.
Urgent At Home same day appointments require a $250 convenience and transportation fee.
Medical Records pick up fee is $20 dollar fee for the 1st set of paper copies and you may also have records placed on a Flash Drive for an extra $10 fee
There is a $35 dollar fee for insufficient check fee
$25 dollar fee for completion of forms for use outside of the patient's medical record, including/but not limited to: FMLA, Written Letters, Jury Duty Letters, Court Documents, Lawyer requests, or other letters requiring the physicians time.
